Your First Visit
Thank you for choosing Southeastern Physical Therapy for your rehabilitation needs. Please arrive 15 minutes early for your first visit to fill out necessary and helpful paperwork. At this time you will be asked to present your prescription (unless it was already sent over by your doctor), your photo ID and proof of insurance. The evaluation will take about one hour. Please wear loose and comfortable clothing; if possible, something that can easily expose your injured area.
Please note: If you are being seen from pelvic floor or incontinence issues, there is a specific pelvic floor medical history form.Below are the necessary forms that you can fill out ahead of time prior to your first visit.
